What is PCOS and can Trimly help?

Polycystic ovary syndrome (PCOS) is a health problem that affects 10% of women in their childbearing years (16 - 49). Women with PCOS have a hormonal imbalance and metabolism problems that may affect their overall health and appearance. PCOS is a common cause of infertility. A common symptom of PCOS is unexplainable weight gain with very little success in losing weight with traditional methods such as diet and exercise. Other PCOS symptoms include hair thinning, unwanted hair growth on the body, acne, mood swings, and more. PCOS is also one of the most common causes of infertility.

In 2014, Dr. Aliabadi started treating PCOS patients for weight loss which helped regulate their periods and made it easier for them to get pregnant. She treated their other symptoms at the same time, relieving them of the pain and stress that PCOS can cause. Dr. Aliabadi realized that doctors were typically treating only the symptoms of PCOS patients and not diagnosing the disease itself. She made it her mission to educate women on PCOS and to help them gain back their confidence so that they could live more fulfilled lives.

What is insulin resistance?

Insulin resistance is a common condition in the United States. In simple terms, insulin is a hormone that regulates the amount of sugar (glucose) in your blood. When you are insulin resistant, your cells don’t work the same way as someone who is not resistant. Glucose that would normally enter a cell and be used for energy stays in your blood and increases over time which can lead to type 2 diabetes and obesity. When insulin levels are high, it’s almost impossible for your body to burn fat. Your body goes into fat storage mode instead. So when you have a high amount of insulin in your blood, your calories will be stored as fat – regardless if you are on a low calorie diet or not.

Trimly will help reduce insulin and will also help improve other hormones in your body reducing symptoms associated with those imbalances.

Medications

What are the medications that Dr. Aliabadi prescribes?

Depending on your weight loss needs, Dr. Aliabadi will create a special treatment plan for you to reach your optimal weight. She will use FDA-approved GLP-1 medication plus system boosting supplements when needed. These medications are non-habit forming.

What are GLP-1 medications and are they safe?

Glucagon-like peptides (GLP-1), originally used to address type 2 diabetes, help improve blood sugar control which in turn leads to weight loss. There are several types of GLP-1s on the market – the first GLP-1 was FDA approved for weight management in 2014 and since then, multiple others have received FDA approval.

How do GLP-1 medications work?

GLP-1s are powerful hormones made by your small intestine that send signals to your brain and other parts of your metabolic system. They regulate your insulin response and regulate your appetite and digestion so you can feel full longer while maximizing nutritional absorption. The medication used in Trimly addresses the fluctuations of blood sugar and facilitates fat loss.

What are the side effects of GLP-1 medications?

Side effects for GLP-1s may include nausea, vomiting, stomach pain, upset stomach, dizziness, constipation, feeling bloated, belching, gas, headaches and fatigue. If you do experience side effects, they usually subside after a few weeks of your body getting used to the medication. Each patient will receive a care package of over the counter and prescription medication to help combat the side effects should they arise along with directions on dosage to help keep side effects to a minimum.

How do I take GLP-1 medications?

Most GLP-1 medications prescribed by Trimly are self-injecting pens, meaning you will receive a pre-filled pen and a pen needle. The injection itself is not painful and you will most likely not even feel it. There are alternatives to GLP-1 self-injecting pens, please talk to your Trimly team member for more information.

When will I get my medication?

Once your prescription is sent to the pharmacy and we get approval from your insurance company, it will ship out within 2 days. However, the insurance company approval process can take anywhere from 1-2 weeks. If your insurance company denies coverage, we can help appeal it for you which may take up to another 2 weeks.

If you are paying out-of-pocket for the medication, it will be shipped out to you by FedEx and you will receive it within 3 days. Some patients prefer to avoid approval times from their provider and opt to pay themselves.

How long am I allowed to take the medication?

Depending on the medication and your treatment plan, you will use the medication as long as it remains beneficial to your weight loss goal and maintenance plan.

What should I do if I feel nauseous or I start to vomit?

We recommend that patients maintain the current dose, or lower the dose until symptoms resolve. Our care package will contain medication to alleviate nausea.

Can I use the medication during pregnancy, while trying for pregnancy, or while breastfeeding?

No. Weight loss medications should not be used in pregnancy or while trying for pregnancy or postpartum while breastfeeding.

Why does my injection site feel bumpy?

A bump at the injection site means the medication was injected just under the skin instead into the fat tissue. You want to pinch on an area of fat, ensure the needle is inserted all the way, and keep the needle injected for a few seconds before pulling it out, so all the medication has been absorbed properly. To prevent bumps, scar tissue, or irritation, rotate injection sites.

Is the Trimly treatment safe?

Trimly uses only FDA approved medication as part of its treatment. With Trimly you are working with a world-renowned doctor who has been treating patients for weight loss since 2014.

Is Trimly available in all 50 states?

Trimly is serving patients in AZ, CA, FL, GA, IL, MA, MI, MN, MS, NC, NJ, NV, NY, OH, PA, TN, TX, WA, WV. We will be available in your state in the near future!

Do I have to follow a specific diet on Trimly?

You do not have to follow a specific, restrictive diet on Trimly, but you will lose more weight if you eat a well-balanced diet full of whole foods, lean proteins, healthy fats and fiber. You’ll create better habits that will also help you keep the weight off.

Can I start and stop the Trimly treatment?

Once you start the Trimly treatment, there are no grace periods during the year of treatment. There are no stops and starts. For example, if a patient is going on vacation, or going away to school, or if a patient gets pregnant, the Trimly treatment will not get put on hold. No exceptions.

How do follow ups get done?

Our team will do our best to schedule you for a follow up through Klara on a monthly basis. If you don’t hear from our team, please reach out to us on Klara or call us at 310-928-6101. This treatment is a partnership and we will need your participation in making it a success.

Do I need to exercise on Trimly?

Our goal is to create healthy habits for long-term health. Exercise is not a requirement, but something we strongly encourage. Even if it’s walking 20-minutes a day. Movement is key to good health and Trimly patients have reported that exercise becomes easier as they start losing weight.

Will I gain my weight back when I’m done with Trimly?

The one-year program will help you lose unwanted weight and put your body in weight-loss mode. Some patients continue Trimly for over a year depending on the amount of weight they want to lose. Your Trimly team will create a maintenance plan for you once you reach your goal weight, but like everything else in life, it’s up to you. We will provide you with guidance and tools to create positive eating and exercise habits for long-term health. Most patients adopt a healthier lifestyle as they start shedding weight because they feel good and want to continue the new habits they have adopted.

What if I stop losing weight?

A Trimly team member will monitor your progress along the way. If your weight plateaus, we will review your history and make any adjustments and/or additions needed at that time. Plateaus are normal during weight loss and will be addressed.

What if I don’t want to do this for a whole year?

This is a year-long commitment. Your body is not a light switch. You need to give it time to get used to the medications and allow them to work to their fullest potential. Healthy weight loss is gradual and as you start to see results, you will be more motivated to continue. Some patients continue beyond a year because they want to lose even more weight. Some patients reach their goal within the first 6 months, and they get on the maintenance plan for the rest of the year.

What happens after I reach my goal weight?

We usually recommend patients to continue their medications for 6 more months after their goal weight is reached. This will help with maintaining your weight.

How much weight should I expect to lose on Trimly?

On average, Dr. Aliabadi’s patients have lost 24 pounds in the first 4 months of her treatment and about 46 pounds in the first year. Some patients have reported losing between 60 to 100 pounds. Patients who eat a well-balanced diet and begin some form of exercise tend to lose more weight than those who do not change their current habits.

What if I get pregnant during the Trimly treatment?

If you are actively trying for pregnancy, please wait until after the pregnancy to begin the Trimly treatment. If you are If you are on the Trimly treatment and find out that you are pregnant, you will need to end the treatment immediately. Please note that Trimly will not issue refunds in this situation.

Eligibility

Who is eligible for Trimly?

  • Patients with a BMI of 27+ with a comorbidity (for example, high blood pressure, high cholesterol, sleep apnea, hypothyroidism, Polycystic Ovary Syndrome (PCOS), cardiovascular disease, pre-diabetes, type 2 diabetes, etc)
  • Patients with a BMI of 30+ without comorbidity
  • *Eligibility requirements may differ between insurance plans

Who is not eligible for Trimly?

  • Anyone under 12 years of age
  • Women who are pregnant or breastfeeding
  • Women who are actively trying to get pregnant
  • Anyone with an eating or substance abuse disorder
  • Anyone with active cancer, active gallbladder disease, a recent bariatric surgery (within the past 12 months), or a history of pancreatitis (in the past six months).
  • Anyone with medullary thyroid carcinoma or a family history of medullary thyroid carcinoma
  • Anyone with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
  • Anyone with Type I diabetes

Insurance/Pricing

Is Trimly covered by my insurance?

The Trimly treatment fee of $2,500 is an out-of-pocket expense, which means that your insurance will not cover this fee. The medications used on the Trimly treatment are billed to your insurance. About 80% of patients who are eligible for the Trimly treatment get insurance coverage on their medication. Some insurance plans exclude weight loss medications from their coverage plan, and therefore, even if a patient meets the requirements for these medications, their insurance companies may not cover them.

How much does the Trimly treatment cost and what’s included?

The year-long treatment costs $2,500* and will include:

  • An initial Zoom with Dr. Aliabadi's trained Physician Associates.
  • Monthly check-ins with our Physician Associates to track your progress and to help you if you reach a plateau.
  • 7-days a week access through our patient portal.
  • Access to webinars created by Trimly coaches on nutrition, mental health, and fertility. We also take requests on webinar topics from our patients.
  • Access to exclusive content in our Patient Resource Center on Trimly.com.

Patients who are approved through their insurance plans for their medication will be responsible for their medication copay, which will be an out-of-pocket expense.

Patients who don’t fall under the eligibility category for insurance coverage of semaglutide will be responsible to pay for their medication which can range anywhere from $650 – $1,400/month. These are market rate prices, and Trimly has no authority over the pricing of these medications. Medication prices may change without prior notice.

Trimly also uses other FDA approved weight loss medications that are not semaglutides and are not covered by insurance. Those medication costs are significantly lower than semaglutides and run between $10 and $150 a month.

Can I use my HSA or FSA account to pay for my Trimly Treatment?

You can use your HSA or FSA funds to pay for your Trimly medication and the treatment.

What will my copay be for the medication

Patients who are approved through their insurance plans for their medication will be responsible for their medication copay, which will be an out-of-pocket expense.

Trimly also uses other FDA approved weight loss medications that are not semaglutides and are not covered by insurance. Those medication costs are significantly lower than semaglutides and run between $10 and $150 a month.

If you're eligible for a refund:

If you meet the eligibility for the medication, and your insurance does not cover the cost of the medication to start Trimly, we will appeal the insurance company’s decision where appeals are applicable. There are different brands within the class of GLP-1 agonists. If one brand is not covered by your insurance plan, we will submit a script for a different brand to help find one that is covered. If they still don’t cover the cost of the medication, you can either pay for the medication out of pocket or receive a refund minus a $100 processing fee.

If you’re not eligible for a refund:

If your BMI is 27 or lower without a comorbidity, insurance companies will not cover the cost of the medication. To go forward with the treatment, you must pay out of pocket for the medication in addition to the Trimly treatment fee of $2,500. You will not be eligible for a refund if you fall into this category since it is already established that insurance plans will not cover your medications at this lower BMI.

Please keep in mind that most patients have a copay for their medications — check with your insurance company to find out what your copay amount is. Your copay will depend on your insurance plan and whether or not your pharmacy deductible has been met. A copay does not mean that your insurance company is not covering your medication, it just means that you have a copay for your medication. You will not be eligible for a refund if your insurance company covers the medications with a copay. 

If your insurance company stops covering your medication after you have started the treatment, or if you change insurance companies while on the treatment and your new insurance company does not cover the medication, you will not be eligible for any type of refund.

If a pharmaceutical company offers a coupon to apply for any GLP-1 agonist and the coupon expires at any point during your Trimly treatment period, you will not be eligible for a refund.

If you are actively trying for pregnancy, please wait until after the pregnancy to start the Trimly treatment. If you are If you are on the Trimly treatment and find out that you are pregnant, you will need to end the treatment immediately. Please note that Trimly will not issue refunds in this situation.

The vast majority of our patients lose weight and reach their goals by following our guidelines when it comes to medication and lifestyle changes. It is extremely rare that a patient may not lose weight on our treatment. Patients that are not happy with their weight loss are not eligible for a refund.

If you begin the Trimly treatment and then decided that you don’t want to continue for any reason, you are not eligible for a refund.

Medications may be on backorder at some point during your treatment. Patients will be given options while they wait for medications to get restocked, but refunds will not be given due to shortages.

What happens if I am eligible for the Trimly treatment but my insurance doesn’t cover the medication?

We encourage people to shop around for new insurance plans during open enrollment. Before signing up for a new plan, make sure you ask them if the specific plan you are considering covers weight loss medication.

The other option is to pay out of pocket for the medication. Losing weight to create a healthier lifestyle is a positive investment in yourself and can save you time and money down the road by helping to prevent health issues that may arise from being overweight or obese.
